Complex Decongestive Therapy

Complex Decongestive Therapy (CDT) is the gold standard for treating lymphedema and consists of an intensive decongestion phase, followed by a longer maintenance phase. CDT consists of 4 main components of treatment: manual lymph drainage (MLD), compression therapy, skin care, and therapeutic exercise.

With AxiomHealth, patients get valuable one-on-one time with an lymphatic specialist for a minimum of 60 minutes in the comfort of their home. Complex Decongestive Therapy treatments consist of:

Manual Lymphatic Drainage (MLD):

MLD is a light, skin-stretching technique that helps promote the movement of lymphatic fluid out of the swollen limb. It should not be confused with a traditional massage. MLD is specifically focused on the lymph vessels to help the flow of lymphatic fluid. MLD is applied to your unaffected areas first, making it possible for the fluid to move out of the affected area, known as “decongesting”. It helps open the remaining functioning lymph collectors and move protein and fluid into them, as well as to help speed up lymph fluid flow through the lymphatics.

Compression:

Compression therapy is the cornerstone of conservative therapy in lymphedema patients. Decongestive therapy is initiated in the active decongestion phase with the use of multicomponent bandage materials of high stiffness, and it should only be applied by trained healthcare professionals. In the maintenance stage, sustained compression over the course of the patient’s life is needed in order to keep edema reduced. This can be done easily with adjustable velcro devices (even in the initial therapy stage.)

Skin Care:

Skin care is an important aspect of managing lymphedema. It helps to recover your skin from compression therapy and to protect from infections. The goal of skin care is to keep your skin intact and maintain its protective layer. Skin changes and infections are common complications with lymphedema and therefore is part of both phase I and phase II. Keep in mind that it depends on your individual skin condition which skin care routine and product is best for you. Your healthcare professional will help you find the right products.

Therapeutic Exercise:

There are many types of physical activity and exercise that can help improve circulation and decrease lymphatic fluid accumulation. Muscles play an important role in “pumping” fluid out of the tissues in the involved areas. These activities can include walking programs, swimming, biking, light resistive exercises, range of motion exercises and deep breathing. Your healthcare provider will help guide you in the proper exercises that are right for you.
